Preparing For Your Appointment

Contact us by phone or fill out the form provided on this site to schedule a sleep apnea consultation in our Lake County IL area dental office.  We offer a no-fee airway evaluation to determine if oral appliance therapy may be applicable to your specific type of sleep apnea.  Please bring a copy of your most recent sleep study on polysomnography to this appointment.  During your one-on-one meeting, Dr. Debora Klein will go over your diagnosis, explore treatment options and ultimately help you decide what next steps to take to combat your sleep apnea.

What Should You Bring To Your Consultation?

• The results of any and all sleep tests you may have taken
• Take note of all the symptoms you are experiencing
• A list of all medications that you are taking
• Investigate what your insurance covers and make sure that your insurance card is available – we can help!
• Ask a family member who has witness you sleeping to attend the consultation with you
• Bring a list of personal information to include recent life changes 
• Bring previous results of other sleep tests you may have taken (if available).
• Bring a list of questions that you may have for doctor
